Today, I’d like to take a deeper look at what actually changes before and after lip filler, using a real clinical case as an example.

Right after the procedure, mild swelling and shine may make the lips appear slightly more pronounced.
However, when you look at the overall shape:
– Central volume becomes more defined
– The vermilion border appears sharper
– Corners of the mouth open slightly upward
As a result, the lower face looks softer, brighter, and more structured.
The line connecting the nose tip, lips, and chin (Ricketts Line) flows more naturally, giving the impression of a sharper, more refined facial contour.
Would this type of lip shape suit me?

In their 20s–30s, many people visit the clinic because:
• Their lips are too thin
• The corners of the mouth turn downward
• Asymmetry is noticeable
In patients in their 40s–50s and older, common concerns include:
• Lipstick smudging due to volume loss
• Vertical wrinkles creating a tired look
Beauty standards vary, but most people find the following “harmonious”:
• The lower lip is slightly fuller than the upper lip
• The width of the lips is not excessive relative to the face
• When gently smiling, the lip corners lift naturally

Still, matching numerical proportions alone does not guarantee a beautiful result.
You must also consider:
– Volume asymmetry
– Mouth corner direction & dimples
– Cupid’s bow definition
– Philtrum length & lower-face ratio
– Chin length, dental alignment, habitual facial expressions
So even if you bring a photo and say, “I want lips like these,”
I reference that atmosphere while designing what is anatomically achievable for your face.
What do we focus on when planning the design?

To create a natural result, I don’t simply add filler at two or three points.
I break the lips down into multiple sub-units and evaluate each.
My design sequence is:

Asymmetry of upper & lower lip volume

Mouth corner & dimple direction

Upper–lower volume ratio (typically 1 : 1.3–1.5)

Cupid’s bow & white roll integrity

Philtrum length, midface/lower-face ratios

Degree of lip eversion (outward roll)
For patients with a long philtrum or long chin, simply adding volume is not ideal.
Instead, slightly everting the upper lip can create the illusion of a shorter philtrum/chin.
For male patients, over-emphasizing the lower lip can look overly feminine,
so I often reinforce the upper-lip structure instead.
Which filler product is best for lips?

Because the lips move hundreds of times a day,
I prefer hyaluronic acid fillers that offer:
• Good cohesivity
• Soft, smooth spread
• A gentle, flexible texture
This type also fades more naturally over time without creating bumps or visible borders.

On the other hand, collagen-stimulating fillers such as Sculptra, Ellansé, and Radiesse should not be used in the lips because:
– They cannot be dissolved
– Rare nodules or granulomas are difficult to remove
For highly mobile areas like the lips,
the top principle is:
“Choose a filler that can be dissolved at any time.”
Side effects — what should you actually worry about?

The lips are not the most dangerous injection area,
but incorrect depth or placement can cause complications.
Too deep
→ The labial artery under the orbicularis oris may be affected,
leading to severe bruising, swelling, or in rare cases, ischemia.
Too superficial
→ Filler may protrude into the white border, causing
a “white, puffy line” when smiling (filler migration).
This often does not resolve on its own and may require dissolving.
Therefore, instead of squeezing a large amount into a shallow layer,
I inject small droplets slowly into the submucosal plane,
building the shape safely layer by layer.
Most patients notice shape changes immediately,
with swelling and shine settling naturally within 2–3 days.
Bruising varies by individual,
but with a fine needle, only about 20–30% experience mild bruising,
which typically fades within a week.
Lip Filler 1cc — What changes before and after?

Lip filler does not dramatically change your entire face.
However, by adjusting:
• Philtrum length
• Mouth corner direction
• Nose–lip–chin alignment
the lower face becomes more defined and balanced.
Even with the same filler product,
results differ depending on:
– Design
– Product properties
– Volume used
– Injection plane
– Rheology (G’, G’’)
Be open about your preferred style and concerns.
